Transition Planning

We actively plan for routine and life transitions.

  • We plan for transitions throughout the day using tools such as visual timetables, planners, and maps, helping learners build the skills to manage change.
  • At key transition points—such as moving to a new class or setting—practitioners actively gather information about each learner to ensure a smooth transition. This includes support strategies, interests, and effective arrangements.
  • Transitions are carefully planned and may include additional visits to new settings.
  • We work in partnership with parents and carers to support their child’s transition, encouraging them to take practical steps at home to ease the process.
